Statement of Privacy

Hôpital de Mattawa Hospital (“Mattawa Hospital”, “we”, “us”) is committed to protecting your privacy and to providing a safe online experience. This Statement of Privacy applies to the Mattawa Hospital 50/50 website (the “Site”) and governs data collection and usage in connection with the Site and the purchase of 50/50 raffle tickets. By using the Site, you consent to the data practices described in this Statement.

Collection of your Personal Information

Mattawa Hospital collects personally identifiable information that you provide when you purchase raffle tickets or contact us, such as your name, email address, home address, telephone number and your confirmation that you are 18 years of age or older and physically located in Ontario at the time of purchase. This information is required to issue your tickets, confirm your eligibility, contact you if you win and keep the records required under our raffle licence (Raffle licence RAF1383227, Alcohol and Gaming Commission of Ontario (AGCO)).

Payment information. Online payment for raffle ticket purchases is processed by a third-party payment processor on its own secure systems. Mattawa Hospital does not store your full payment card number; we receive confirmation of payment, the amount, the last digits of the card and a transaction reference.

Winner verification. If you win, we collect details of government-issued photo identification in order to verify your identity, age and Ontario address before a prize is released.

There is also information about your computer hardware and software that is automatically collected by Mattawa Hospital. This information can include your IP address, browser type, device type, domain names, access times and referring website addresses. This information is used by Mattawa Hospital for the operation of the Site, to maintain quality of the service, to provide general statistics regarding use of the Site and, together with your device's location where you allow it, to confirm that you are located in Ontario at the time of purchase, as required by our raffle licence.

Mattawa Hospital encourages you to review the privacy statements of any website you choose to link to from the Site, including the payment processor and ConnexOntario, so that you can understand how those websites collect, use and share your information. Mattawa Hospital is not responsible for the privacy statements or other content on websites outside of the Site.

Use of your Personal Information

Mattawa Hospital collects and uses your personal information to operate the Site and deliver the services you have requested: to sell and deliver raffle tickets, send purchase confirmations and ticket numbers, verify eligibility and prevent fraud or ineligible purchases, conduct the draw, contact and verify winners, pay prizes and publish draw results (winner name or initials, community, winning ticket number and prize amount, as permitted by the Rules of Play).

Mattawa Hospital may also use your personally identifiable information to inform you of future 50/50 draws and other fundraising activities of Mattawa Hospital, where you have agreed to receive such information. You may withdraw this consent at any time using the unsubscribe link in any email or by contacting us.

Mattawa Hospital does not sell, rent or lease its purchaser lists to third parties. Mattawa Hospital may share data with trusted partners to help us operate the raffle and the Site: the electronic raffle system supplier that generates ticket numbers and conducts the draw, the payment processor, the email delivery service, and website hosting and analytics providers. All such third parties are prohibited from using your personal information except to provide these services to Mattawa Hospital, and they are required to maintain the confidentiality of your information. Some providers may store or process information outside Ontario or Canada.

Mattawa Hospital does not use or disclose sensitive personal information, such as race, religion or political affiliations, without your explicit consent.

Mattawa Hospital keeps track of the pages our visitors view within the Site in order to determine which content is most useful. This data is used to improve the Site. We do not use it to deliver advertising.

Mattawa Hospital will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law, the conditions of our raffle licence or requests of the AGCO, or comply with legal process served on Mattawa Hospital or the Site; (b) protect and defend the rights or property of Mattawa Hospital; and (c) act under exigent circumstances to protect the personal safety of users of the Site or the public.

Retention of your Personal Information

Mattawa Hospital keeps raffle purchase and draw records for the period required by the AGCO (at minimum four years after the draw) and as necessary for accounting and legal purposes. Winner identification is kept only as long as needed to verify and pay the prize and satisfy licence requirements. Records of marketing consent are kept until you withdraw consent.

Use of Cookies

The Site uses “cookies” to help you complete your ticket purchase and to personalize your online experience. A cookie is a text file that is placed on your device by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you and can only be read by a web server in the domain that issued the cookie to you.

One of the primary purposes of cookies is to provide a convenience feature to save you time. For example, a cookie keeps your ticket purchase session active while you complete checkout, remembers the choice you made in our cookie banner, and helps us understand how the Site is used so we can improve it.

You have the ability to accept or decline cookies. When you first visit the Site, a banner lets you accept all cookies or only those that are essential. Most web browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer. Security of your Personal Information

Mattawa Hospital secures your personal information from unauthorized access, use or disclosure. Mattawa Hospital secures the personally identifiable information you provide on computer servers in a controlled, secure environment, protected from unauthorized access, use or disclosure. When personal information (such as a payment card number) is transmitted to the payment processor or other websites, it is protected through the use of encryption, such as the Transport Layer Security (TLS/SSL) protocol.

Access to and Correction of your Personal Information

You may ask to see the personal information we hold about you and request corrections by contacting us using the details below. We may need to verify your identity before responding. Please note that we cannot delete purchase or draw records that we are required to keep under our raffle licence.

Children

The Site and the raffle are intended only for persons aged 18 and over. Mattawa Hospital does not knowingly collect personal information from anyone under 18. If you believe a minor has provided us with information, please contact us and we will delete it.

Changes to this Statement

Mattawa Hospital will occasionally update this Statement of Privacy to reflect changes in the raffle, the Site or applicable requirements. Mattawa Hospital encourages you to periodically review this Statement to be informed of how Mattawa Hospital is protecting your information.
